Broker’s Authority Reinstatement Guide

To successfully reinstate your operating authority (MC number), please carefully follow the steps outlined below:

Step 1 : Ensure Your USDOT Number is Active and Up-to-Date

  • Before you can reinstate your operating authority, your USDOT number must be active and all associated information must be current. You can verify the status of your USDOT number and update your information as necessary here.

Step 2: Request Your USDOT PIN

If you don’t have your USDOT PIN, you will need to request it by mail. Follow these steps:

  • Select “Yes” and enter the required verification information.

  • Upon successful submission, you will receive a confirmation screen indicating that your USDOT PIN has been mailed to you.

Step 3: Reinstating Your Operating Authority (MC Number)

Once you receive your USDOT PIN, proceed with your reinstatement request online:

  • Select the option: “Requesting that my Operating Authority (MC number) be reinstated” (this is the 10th option on the page).

  • If you’re unable to submit your request online, complete the MCSA-5889 Form, available here. Ensure sections A, D, and E are fully completed.

Note: To safeguard your information, the company official signing the form must include a copy of their Driver’s License. Submit your completed form via the “Send Us a Ticket” option here, or fax it to 202-366-3477.

Step 4: Reapply for Operating Authority

If your operating authority was permanently dismissed, follow these additional steps:

  • After receiving your USDOT PIN, navigate to the USDOT update page and add “Authorized for Hire” to your existing USDOT number.

  • Apply for operating authority by selecting “I need an additional Operating Authority to my existing USDOT number.”

Ensure your insurance company electronically files the required insurance, and a process agent files the required BOC-3. You can find a process agent here. Remember, there is a $300 fee for each operating authority requested, and this fee is non-refundable, so please choose the correct type.
